HCP Patient Portal

The St Margaret’s HCP Portal enables Health Care Professionals remote access to a range of services on a 24/7 basis. The Portal has a direct link to the Crosscare Clinical Database used within the hospice by all clinical and administration teams.

Services accessed by registered Healthcare Professionals will include:

  • E-referrals for hospice and lymphoedema services. This will not replace the existing CRC (Central Referral Centre) referral process but can be used as an alternative to speaking to our referral staff or when a referral is made out of normal office hours.
  • A pre-defined selection of the patient medical records
  • Patient questionnaires or assessments
  • View and edit patient demographic information
  • View courses and training videos that may have been prescribed to the patient by one of the hospice’s Healthcare professionals.
  • View a list of patients on our In Patient and Day Care units.
  • View appointments for the current user logged in for today
  • Administer Caseloads

Registering to access the portal

To access these services, no specialist equipment or software is required, only access to a web browser with an internet connection.

To sign up for this service click this link and register now as a healthcare professional.

You will be asked to provide contact details and the name of your team or GP Surgery and where you are based, which will be verified before access is given.

After the “Health or Social Care Professional Enquiry From” has been submitted, St Margaret’s will contact you within a few days to complete a short training session, after which your Username and Password will be provided.

Once a healthcare professional has completed the initial registration and online training it allows them to send referral information to us quickly and easily. The benefit for the team here at St Margaret’s is that it allows us to keep our lines open and available for our patients and their families whilst also speeding up the referral process. This in turn allows our clinicians to triage a referral immediately and without further need to liaise.
