Our Impact

Thanks to the generous support of our community, we were able to provide care and support for almost 5,000 people in our community last year. In addition, our work and services touched the lives of thousands more people, through our community engagement and fundraising events, education, and training activity as well as through the added social value delivered by our charity shops and volunteering programmes.

Our services are available free to anyone, regardless of their background, faith, culture, personal identity, or location within the county. We firmly believe that compassionate care should be accessible to all, and we strive to ensure that our support reaches every individual and family who can benefit from it. Crucially, we also recognise that this means looking after those closest to them – the husbands, wives, life partners, children, and carers of our patients, both before, during and after death.

Last year we:

Cared for 5,680 people, including end of life and Lymphoedema patients

plus family members and carers who benefited from our wraparound care such as family support, bereavement and spiritual care.

that came through to our nurse-led 24-hour Advice Line and Central Referral Centre, helping patients, carers, families, and healthcare professionals who need specialist palliative care advice.

Welcomed 213 patients to our Sunflower Centres (day hospices)

in Yeovil and Taunton, helping more patients live well for as long as possible.
For those well enough to live at home but travel, our day hospices provide a range of services and support groups, including social activities such as arts and crafts, exercise, and music, where people can come together and share experiences.

1,662 people were cared for in the comfort of their own home

Community services consist of Clinical Nurse Specialists (CNS) and Multi-Disciplinary
Team Assistants (MDTA).
Divided into East and West they mirror Primary Care Neighbourhoods providing care to the most complex patients within their home settings. A total of 1,847 referrals were received resulting in 24,666 patient related activities.

Cared for 213 patients on our In-Patient Unit

providing specialist palliative care, including complex symptom management. Our team supported many patients to return home, if that was there wish, once their pain and symptoms were under control.

Supported 245 family members and carers

offering guidance and compassion at the most challenging times. Supporting those closest to our patients is a key priority, pre, during and following the death of their loved one.

Provided Bereavement support for 167 people

this year, helping individuals and families cope with grief through one-to-one counselling and family sessions.

Our Sunflower Centre Day Hospices provided personalised, holistic care to 238 patients
