What is this chart for?

- Red – Severe symptoms that do not improve after two doses of medication or intervention.
- Amber – Symptoms are present but not severe.
- Green – No symptoms detected.

Anyone providing care can ask about or document the symptoms listed on the chart. If symptoms appear, follow the existing care plan, which may include repositioning, administering medication, or seeking assistance.
Do I have to use it?
Using the chart is entirely optional. At home, it can be filled out as frequently or infrequently as needed to keep everyone involved in care informed about symptom management.
Do I need training?
No formal training is required. However, if you have any questions about how to use the chart, you can reach out to the local nursing team that introduced it.
Who should I inform about symptoms?
